You might want to explain how you have developed skills, qualities, and motivation that have contributed to your achievements, and you could link these experiences to the course or subject area you’re interested in. Consider how whether you have gained insight, or developed aspects of your character which make you stand out from other applicants.
However, sharing information about your disability in this way is entirely your choice, and you are not expected to talk about it if you don't want to.
